PRIVACY & CONFIDENTIALITY POLICY

PURPOSE OF COLLECTING AND HOLDING YOUR INFORMATION

As part of providing a coaching & consulting service, Urvashi Lalchandani will need to collect and record personal information. This includes personal details and a record of coaching & counselling sessions. Please be aware all information may be stored electronically, including emails or a record of phone conversations. ‘Keeping notes’ is a necessary part of the coaching & counselling process. You do not have to give all your personal information.

HOW YOUR INFORMATION IS COLLECTED

Your personal information is collected in several ways before, during or after coaching & counselling consultation with Urvashi Lalchandani. For example, when you make contact through Urvashi’s website; correspond with her via email; interact during session or during telephone conversations with Urvashi, or via referrals and correspondence.

HOW YOUR INFORMATION IS STORED

Client files are stored securely in electronic form. Hand-written notes are uploaded to electronic storage on the day of consultation, then securely shredded.

REQUESTS TO ACCESS INFORMATION

You may request access to file information at any stage. We require that all such requests are made in writing by you. Urvashi may request that you make an appointment to discuss the contents of the personal information and the reasons for the request. This discussion helps to ensure that your personal information is appropriately managed and accessed. Access to personal information kept on file is subject to exceptions in the relevant legislation.

DISCLOSURE OF CLIENT INFORMATION

Urvashi will not disclose any of your personal information except when:

– A court subpoenas it; or

– Disclosure is otherwise required or authorised by law; or

– It is Urvashi’s reasonable belief that failure to disclose the information would place you or another person at serious risk to life, health or safety; or

– Your prior approval was obtained to: provide a written report to another professional or agency. e.g., a GP or a lawyer; or to discuss the material with another person, e.g. a parent, health provider or employer; or to disclose the information in another way.

CLIENT AGREEMENT

I have read and understood the above Consent Form. I agree to these conditions for the coaching & counselling services provided by Urvashi Lalchandani.

I give permission to Urvashi Lalchandani as my Coaching & Counselling Consultant to obtain and/or release my personal information from and/or to the following individuals or agencies:

Emergency contact

Email consent: I consent to Urvashi Lalchandani sending a copy of this agreement to my email address provided above. I further consent to receiving reminders, invoices and other relevant communications to the email address provided.

CODE OF CONDUCT

TheYellowFish – Coaching & Consulting prides itself on its ethical standards and all life coaches are members of the International Coach Federation (ICF) and are bound by the ICF Code of Ethics. In accordance with ICF requirements, TheYellowFish has established the following guidelines to ensure the interests of both the coach and coachee are met within coaching sessions.

BEFORE THE INITIAL COACHING SESSION:

Before you commence with the coaching process you will be asked to read the following documents:

– Code of Conduct
– Practice Information Sheet
– Coaching Agreement

AT COACHING SESSIONS, YOUR COACH WILL:

– Always operate in a professional manner
– Be open and honest about his/her level of coaching skills
– Establish a coaching agreement with you within the first coaching session
– Respect your right to confidentiality
– Avoid dual role conflicts
– Avoid accepting money for referrals from any professional to whom you are referred
– Alert you to the fact that coaching is no longer necessary when that time arises
– Always honour the coaching profession

ONCE YOUR COACHING IS COMPLETED YOUR COACH WILL:

– Contact you in due course to find out how you are progressing
– Ask if you can be used as a reference (if required)
– Respect your right to confidentiality

THE RESPONSIBILITIES OF A COACHEE

Coachees have a few responsibilities as part of the coaching process. As a coachee you will be required to actively engage in the coaching process and:

– Talk openly and honestly about your goals and circumstances
– Actively participate in the process and work with the coach
– Be willing to explore possible solutions and take action towards achieving your goals.
– Attend all scheduled meetings ready and prepared to engage in coaching
